One strategy for assuring quality health care in the rural region served by UM Shore Regional Health is reaching patients “where they are” – going out into the community with information and support services that help individuals and families maintain their best health outside the walls of the hospital. The Mobile Wellness Team (MWT), based at UM Shore Medical Center at Chestertown, does just that!
Led by Nurse Coordinator Emily Welsh, BSN, RN, the four-member team (including Welsh, two community advocates and a social worker) is ramping up to achieve a high-profile presence throughout Kent and northern Queen Anne’s counties. Working with community partners, the team will be on site at farmer’s markets and public events (such as such as the annual Chestertown “Tea Party”) to help locals understand and avail themselves of strategies for better health.
Equally important, all providers in Kent and northern Queen Anne’s counties are encouraged to view the MWT as a resource for them and their patients. Services the team offers include:
- Home safety inspections
- Blood pressure checks and health screenings
- Case management for those with chronic illnesses including diabetes, heart failure, COPD, etc.
- Wellness activities
- Arrangement of telehealth consults with physicians and other clinicians
- Social visits for the homebound
- Connections to community resources
